ConcertoHealth invites the Lynnwood community to an open house and benefit fair at the neighborhood Care Center located at 2322 196th St. SW., Ste. 100.

Attendees can take a tour of the recently opened Care Center, meet the Care Center team and learn how ConcertoHealth improves access to healthcare for Lynnwood’s patient population, including Medicare beneficiaries and dual-eligible Medicare/Medicaid patients.

Open house attendees can also meet and take pictures with Blitz, the Seattle Seahawks’ mascot, and Dr. Cleo, Molina Healthcare’s mascot.

Providers will be on hand at the event to answer questions, from transportation to pharmacy needs. Light snacks and refreshments will be served and attendees will also have an opportunity to win some great raffle prizes. Free gift bags will be given while supplies last.

ConcertoHealth’s care model provides patients with healthcare services where and when they need them, eliminating the most common barriers to appropriate and timely care.
