CONCERTOHEALTH HOSTS OPEN HOUSE FOR LYNNWOOD COMMUNITY

ConcertoHealth invites the community of Lynnwood, Washington to an open house and benefit fair at their new neighborhood Care Center. Attendees can take a tour of the recently opened Care Center, meet the Care Center team and learn how ConcertoHealth brings improved access to quality healthcare for Lynnwood’s patient population, including Medicare beneficiaries and dual-eligible Medicare/Medicaid patients.

For extra fun, Open House attendees can meet and take pictures with Blitz, Seattle Seahawks’ mascot and Dr. Cleo, Molina Healthcare’s mascot. Providers will be on hand to answer questions, from transportation to pharmacy needs. Light snacks and refreshments will be served and attendees will also have an opportunity to win some great raffle prizes. Free gift bags will be given while supplies last.

ConcertoHealth’s Care Model provides patients with healthcare services where and when they need them, eliminating the most common barriers to appropriate and timely care. By merging social work with care management, Concerto improves access to primary care services by building Care Centers in the communities it serves, addressing both the social and economic realities these patients face.
