ConcertoHealth, located at 2322 196th St. SW., hosted an open house Saturday to show neighbors what services are available through the organization.
Attendees took tours of the recently opened Care Center, met the Care Center team and learned how ConcertoHealth improves access to healthcare for Lynnwood’s patient population, including Medicare beneficiaries and dual-eligible Medicare/Medicaid patients.
Open house attendees also met Blitz, the Seattle Seahawks’ mascot, and Dr. Cleo, Molina Healthcare’s mascot.
ConcertoHealth’s care model provides patients with healthcare services where and when they need them, eliminating the most common barriers to appropriate and timely care. By merging social work with care management, Concerto improves access to primary care services by building Care Centers in the communities it serves, addressing both the social and economic realities these patients face.
Through its Lynnwood location, ConcertoHealth provides primary care services to those who can get to the clinics and in-home services for those who can’t. ConcertoHealth also has field-based care teams that can follow members through any other healthcare setting, from hospitals to nursing homes, rehab and more.
